We are clear that a merger is the best way forward, and we want to ensure patients at both practices are fully informed and can have their questions or concerns answered. 

There is a 6 week engagement programme where patients and local stakeholders can ask questions or share their views about the proposed merger. 

This consultation period will begin on 1st May.

Online

Patients registered at the practice will be sent a link to an online survey form via SMS or email. Alternatively click the link below to complete the form.

Paper Versions

Where patients do not have a mobile registered with us, we will post a paper version of the survey.

Copies of the paper version will also be available in both surgeries.

Paper versions of the survey should be returned to Glenpark Medical Practice or Teams Medical Practice reception.

In Person

We will be having a series of face to face drop-in sessions. Details will be posted here when they are confirmed.

Please do not make an appointment at the surgery or call reception to discuss the merger. This is to ensure that during this consultation we can still concentrate on caring for our patients.
